# Appendix B: Reusable Templates

<a id="ministry-charter"></a>

## Ministry Charter

- Name and one-sentence purpose.
- Received-life good served and DDF coverage.
- Governing Scripture and tradition-specific authority.
- People served and people likely to bear risk or hidden labor.
- Owner, backup, reviewer, supervisor, and complaint route.
- Qualification, training, access, child or vulnerable-person controls.
- Budget, money handling, property, data, records, vendors, insurance, and legal review.
- Ordinary rhythm, handoffs, capacity limit, and emergency route.
- Evidence of fruit, possible harm, and review date.
- Pause, transfer, and closure conditions.

## High-Stakes Decision Record

- Exact decision and what is outside scope.
- Authority and participants.
- Claims separated by kind and their source contact.
- Options considered, including delay, partnership, or no action.
- People affected and burdens carried.
- Conflicts declared and managed.
- Safety, doctrine, law, finance, care, and public-witness implications.
- Decision, reasons, dissent if recorded, owner, date, and communication.
- Evidence or event that requires review.

## First-Response Card

> Immediate danger or urgent medical need: contact local emergency help. Possible suicide or self-harm: ask directly, stay present, reduce immediate danger where safe, connect to the current local crisis path, and follow up. Disclosure or allegation: receive seriously; do not investigate or ask leading questions; do not promise secrecy; record the person's words and time; preserve material; contact the current safeguarding and statutory path; bypass anyone implicated; protect against retaliation. Do not: mediate abuse as conflict, force reconciliation, diagnose, announce a verdict, contact an alleged perpetrator without direction, or protect the launch, leader, or institution over people and truth.

Place actual local names and numbers on the operational card. Review them at least annually and whenever law, services, leaders, or locations change.

<a id="quarterly-reality-review"></a>

## Quarterly Reality Review

- Pray and read the governing Scripture in context.
- Name one domain, not "the whole church."
- State what the church says should happen.
- Observe what actually happens from several positions.
- Separate source, channel, receiver conditions, and fruit.
- Name failure mode, burden, and immediate protection.
- Choose one repair, owner, due date, and evidence.
- Tell affected people what will happen and return on the promised date.

## Leader and Household Load Review

Ask privately and without the leader's authority controlling the answer:

- What work is visible, and what work is hidden?
- Which boundary has become hardest to keep?
- What is ministry costing body, sleep, household, friendship, prayer, finances, and joy?
- Is anyone afraid to say no, leave a role, or ask for help?
- What should stop, rotate, be paid, be referred, or be carried by the wider body?
- Who outside the plant remains available for care?
